Activities are undertaken only when included in the person’s care plan and appropriately delegated by a regulated healthcare professional where required. The delegating professional retains responsibility for clinical monitoring and review, supported by ICO’s nurse-led governance.

We use person-specific risk assessments, written guidance, competency records, escalation arrangements and regular reviews.

We work with individuals, families, GPs, community nurses, specialists, therapists and commissioners to coordinate safe support.

Our Complex Care and Delegated Healthcare Support

Inclusive Care Options supports people at home with person-specific delegated healthcare activities. Depending on assessment, the care plan and safe delegation arrangements, support may include:

 

Medication support — prescribed medication regimes, administration and injections where included in the care plan and appropriately delegated.

 

Wound-care support — following written wound-care plans, completing delegated procedures, monitoring and escalating concerns.

 

Respiratory and tracheostomy support — tracheostomy care, ventilation support and monitoring under person-specific arrangements.

 

Nutrition and hydration support — PEG feeding and nutritional support in accordance with the individual’s care plan.

 

Catheter care — support with indwelling and suprapubic catheters under written guidance.

Condition-Specific Support

 

Diabetes — insulin administration, blood-glucose monitoring and recording as set out in the care plan.

 

Epilepsy — seizure monitoring, prescribed treatment and individual emergency protocols.

 

Bowel management — person-specific programmes delivered under written guidance.

 

Pain management — observation, recording, prescribed support and prompt escalation of concerns.

 

Multiple sclerosis and motor neurone disease — tailored support based on the person’s assessed needs.

 

Post-operative support — assistance with recovery and rehabilitation under the agreed care plan.

 

Palliative and end-of-life support — compassionate support delivered in partnership with the responsible healthcare professionals.

 

Availability depends on individual assessment, documented delegation where required, task-specific training, competency sign-off and ongoing clinical review. ICO will only accept activities that can be delivered safely within our registration and governance arrangements.

Ryan Spears is the Clinical Lead at Inclusive Care Options. As an NMC-registered nurse, Ryan provides nurse-led governance for the organisation’s complex-care support, including care planning, staff training, competency systems, review and escalation.

 

Person-specific delegated healthcare activities are delivered under the individual’s care plan and appropriate delegation arrangements. Ryan works with individuals, families and external healthcare professionals to coordinate safe, person-centred support.

 

Ryan’s experience includes NHS, private and community healthcare settings, together with supporting people with learning disabilities and complex needs.
