We ask about your goals, health, routines, risks and communication needs.

4. We check whether our service is right for you.

We support adults aged 18 to 65.

This includes people who are 65.

We usually work in Hammersmith and Fulham, Westminster and Barnet.

We may be able to support people in other areas.

5. If we can support you, we make a plan with you.

We agree the support, the times and a start date with you.
