Referral information for social workers, commissioners and health professionals

Who we support
We provide adult social care for people aged 18–65 inclusive who may have learning disabilities, autism and complex needs. We currently work across Westminster, Hammersmith & Fulham and Barnet. Other areas may be considered case by case.

Our scope
Inclusive Care Options is registered with the Care Quality Commission for Personal Care. We are nurse-led, but we do not provide a standalone nursing service. Day-to-day support is delivered by trained and competency-assessed care workers. Appropriate delegated healthcare tasks are undertaken only when included in the person’s care plan and supported by clear professional instructions.

Making a referral
Please include the person’s needs, communication preferences, desired outcomes, risks, current professionals, proposed support hours and funding status. We assess each referral individually and agree whether we can safely meet the person’s needs before support starts.

What happens next
We aim to respond within one day. Our office is open 9am–6pm, Monday to Sunday. Existing customers can access telephone nurse support outside these hours. New referrers should use the main number or referral form.
